We are currently trialling a new way of conducting your Annual Asthma Reviews.
Some of you may receive a text message or email from us asking you to complete a secure online questionnaire about your Asthma.
The answers that you send back to us may mean that we will not ask you to attend the Surgery for a Face to Face Review.
Your responses remain entirely confidential and will be added to your Medical Record to help our Team provide the appropriate treatment, medication and support to you.
If you receive one of these messages, please take the tine to complete the Questionnaire, and should you have any concerns please contact the Surgery.
